Elders Equine Veterinary Service offers our patient form(s) online so you can complete it in the convenience of your own home or office. The new patient and additional horse forms can be filled out and submitted online while the transfer of records form is a pdf that you can fill out and fax in or provide at the time of appointment.

This let's us record our pet and owners into our management software so that we can effectively and efficiently provide the service you deserve! Please fill out Veterinary Service Agreement and Payment Policy form below. If you have additional horses that you would like to have Elder's Equine care for please fill out the Additional Horse Form.

Transfer of Records

If you require records transferred from one veterinarian to another, you can now use the following form to conveniently request the files. The form is provided by the MVMA and is the official form for transfer of documents between veterinarians. In most cases, the files should be transfered within 3-5 days. If you have any questions, feel free to contact us or the MVMA.
